FOR NEW STUDENTS:

  1. Grade 12 Report Card
  2. Certificate of Good Moral Character (given by school head)
  3. Original Copy of PSA Birth Certificate
  4. Your 1 x 1 Photo, scanned (for submission 4 pieces are needed)

FOR TRANSFEREES:

  1. Transcript of Record
  2. Honorable Dismissal
  3. Birth Certificate
  4. Certification of Good Moral Character
  5. Your 1 x 1 Photo, scanned (for submission 4 pieces are needed)

NOTE:

You may submit the original copies once the school is allowed to accept face to face enrolment.

Due to the Enhance Community Quarantine, all OLD students will be considered for REGULAR classes pending the release of Permanent Record for advising.
