Admission Requirements

Students are responsible for the submission and completion of required documents and forms by the concerned offices. Non-compliance or non-submission of documents and forms would constrain offices to regulate the student’s ID or place his clearance or enrollment status on hold. These are:

College Students

Freshmen:

1) Senior High School Report Card (Grade 12)

2) Certificate of Good Moral Character

3) Original copy of PSA Birth Certificate

4) Four (4) ID pictures 2″x2″ (white background, with name)

Transferees and Second Coursers:

1) Transfer Credential | Honorable Dismissal

2) Certificate of Good Moral Character

3) Certificate of Grades | TOR for Evaluation Purposes

4) Original copy of PSA Birth Certificate

5) Four (4) ID pictures 2″x2″ (white background, with name)

ISHRM SHS & TVET Department:

1) Card Original & Photocopy

2) PSA Birth Certificate Original & Photocopy

3) Diploma Photocopy / Certificate Of Completion

4) ESC Certificate / QVR Certificate

5) Four (4) ID pictures 2″x2″ (white background, with name)
